Machine-filled blank form. 11.3x15.4 cm. A typed note on the upper margin: "2. Ausfertigung". The document was valid until April 30, 1942, it was prolonged until July 31, 1942. The document was accompanied by two 1 zloty revenue stamps and the appropriate stamps. Traces of folding, corner loss, edge tears.
